The step wise procedure for grant of wholesale drug Licences is given as below :
Step 1 : The applicant seeking grant of wholesale Licence/s submits an application on plain paper before the Licensing Authority alongwith qualification certificate/s & experience certificate of competent person.
Step 2 : The experience certificate is got verified by the concerned area inspector / or the inspector under whose jurisdiction the firm issuing certificate falls as the case may be.
Step 3 : After receipt of genuineness regarding experience certificate , the Licensing Authority / empowered committee conducts an interview of the applicant / competent person as the case may be.
Step 4 : After successful completion of step 3 , the applicant submits application on Form-19 accompanied by a fee of Rs three thousand alongwith below mentioned statutory / supportive requirements before the Licensing Authority :
- Constitution of the Firm / Photoproforma (Proprietorship / Partnership) & mentioning the full particulars of competent person.
- Documentary evidence in respect of the ownership or occupation on rental or other basis of the premises.
- Site Plan of the Premises ( Minimum carpet area 10 square meters )
- Affidavit/s in respect of Proprietorship / Partnership.
- Agreement deed in case of hiring of competent person
- Two passport size Additional Photographs of competent person/s to be pasted on Licence/s.
- Stockistship Letter.
- State Subject.
In case the applicant / competent person is valid registration certificate holder issued by President J&K Pharmacy Council , then step 1 to 3 is omitted.
Step 5 : The documents complete in all respects as per step 1 to 4 is forwarded to concerned area inspector by the Licensing Authority for on spot inspection of the premises.
Step 6 : The concerned area inspector inspect the premises & submit the above completed documents alongwith Inspection report to the Licensing Authority with the recommendation as : Licence/s may / may not be granted in favor of the applicant alongwith reasons.
Step 7 : The Licensing Authority after being satisfied with the recommendations of concerned area inspector for grant of licence issue the Licence/s in favor of the applicant on Form 20-B / 21 -B
